Model Consent Form

In this image I explained the process of this NEA to my model and the process of taking the photos. My Model is fully aware her pictures will be only used by me and for the purpose of NEA only. At any time she feels uncomfortable she has the full right to withdraw from the process. Both me and my  Model are happy with going through this process.
